Strange IC660 problem

I have an icecap 660 in a 2 lamp configuration that will fire up VHO T12's with out a problem. It will not fire up T5's though. One lamp lights for about a second (blue/orange), the other (yellow/red) never blinks. I know it's not the harness because I have 2 more of these running on my tank and I swapped this with 2 known working harnesses and lamps. It doesn't work on either of those?
Anyone have any insight to this mystery?

Sounds like the ballasts internal protection is turning the ballast off, possibly a bad T5 lamp is causing the problem. Please try the attached test, it requires an electrical meter.

I would agree except I have literally tried with 3 different wire harnesses, 2 of which are in service with 4 brand new (less than a week) T5 lamps. I have tried this ballast on 3 wire harnesses, 12 different T5 end caps and 16 differant T5 lamps (4 are the brand new ones. The other 2 ic660 ballasts I have are being used on my tank and will fire up all the T5 lamps in any combo. I literally have one of the other ballasts running, unplug it, plug this one in and it fails. I unplug it, put the other back on and it works every time.
As soon as I wire in the T12 end caps with very old VHO lamps they light right up every time?
